Admission Procedures

Upon reference by a physician, La Maison Au Diapason welcomes all patients at the end of life regardless of age, religion, origin, culture, education, values or language. Services are provided to residents of the Brome-Missisquoi and Haute-Yamaska communities, or to those who have a significant loved one residing one of these two territories.

La Maison Au Diapason’s admission committee is comprised of the Medical Director and the Director of Nursing. Referrals are evaluated and prioritized based on admission criteria (prognosis, performance scale PPSV2, social situation, etc.).

Relocation

La Maison Au Diapason is a community hospice whose mission is to offer palliative care to patients at the end of life. La Maison Au Diapason is not a long-term care facility. The patient’s condition is assessed regularly and should it stabilize or improve, the medical team will evaluate the possibility of having to consider an alternative resource. The patient may be referred to a centre that is better suited to his needs or possibly return home with support from the CLSC.
